This document is a customizable waiver you can get parents/guardians to sign before your children’s yoga class. This is strongly recommended.
It comes in three formats: .docx Word, plain text, and PDF, compressed as a zip file.
This waiver includes:
* Acknowledgement of risks
* Voluntary participation – confirming the child is in good health and has been briefed about following the instructor’s guidelines and respecting their own physical limitations.
* Assumption of responsibility
* Space for medical information/consent for emergency medical attention
* Optional photo and video-release
